Human inaccuracy is measured a causative feature in 70% to 80% of all aviation accidents. Most slips appear to have been exposed by the crew before they could lead to a problem worth reporting. Our findings propose a need for more effective feedback in support of data-driven monitoring, especially in the case of errors of omission and for shared information of intent amid airborne and ground-based operators to promote the more timely and dependable discovery of mistakes.
