Overall, a variety of health indicators show that Canadians enjoy a standard of health that is among the highest in the world. Canada ranks second in life expectancy, behind only Japan. Canadian mortality rates are among the lowest in Organization for Economic Cooperation and Development (OECD) countries, behind only those of France, Sweden and Australia.

A good health care system will involve consumers, that is, knowing not just about the epidemiology of a population but also knowing more about the individual's perceived needs and preferences including non-health motivators. The Change Foundation report Are Patients becoming Consumers found that there are significant factors in the perception of health care that health care organizations should consider: that health is the dominant concern of all age groups, that many individuals first and foremost see themselves as responsible for their health, and that individuals are more actively taking control of their health care. Organizational behaviors and consumer behavior can provide wider understanding of the ever-widening gap in the demand and supply of health care (Gratzer, 2002).

Although full control by consumers over their health care delivery has traditionally been seen as impractical where health care is publicly insured and where there are limited resources, over time a new perspective may emerge: one of a consumer-driven approach. "Canada needs a clear national vision of health-related attitudes and expectations of its citizens," writes one Canadian author.

Health care in Canada has long been a source of national pride. Known as 'medicare', the system is publicly financed but privately run, it provides universal coverage and care is free at the point of use. The system is based on five founding principles. Care must be universal, portable, comprehensive, accessible, and publicly administered. But does medicare adhere to these principles? Many think not (Gratzer, 2002).
