Stereotypical images of nurse leaders tend to center on the historical concept of the head nurse or director of nursing in a hospital. Although throughout the history of American nursing these nurse leaders had varying degrees of power and authority within hospitals, they were mostly perceived to have more informal authority, with administrators and physicians having the formal governing authority. With only this traditional hospital-based role as an avenue for career advancement, nurses had limited promotional opportunities in the past. If a nurse was seeking career expansion or a change in work outside of the intense, technological, clinical arena, usually teaching or research were the only alternatives. And even these options rarely went beyond the nursing sphere into other health care areas.
So, the executive advancement track was limited to the traditional director of nursing role which was usually filled by a mature nurse who was seen as controlling, autocratic, and entrenched in the bureaucratic organizational structure of hospitals. This person was typically not viewed as progressive or innovative. The mantra of management at that time was planning, organizing, controlling, and leading with an emphasis on controlling. For many nurses, an image comes to them of someone in their past who fit this stereotypical image. The traditional hierarchy of the hospital, rooted in its religious and military history, often led to territorial behaviors and the creation of fiefdoms and turfs. These spheres of influence still affect many hospitals today and are a direct cause of higher costs and barriers to teamwork and the development of multidisciplinary models of care delivery. The director of nursing was often seen as a powerful figure because of the central and integrating role that nursing played throughout the organization. Although this central, coordinating role led to a sense of pervasiveness of nursing in hospital structures.......
